1. Person or Entity to Serve - We need to increase the characters on teh Person or Entity to Serve field to 190. Rare to see so many but should solve issues if one gets big.
2. The box arond the Last Day and Special instructinos looks good, but we do not need dott lines above and below the box.
3. remove dotted line below capacity.
4. we still need to find room, i see 1/2 inch at bottom and top of form. Plus with the removal of the dotted lines noted above we should be able to fit all on one page.
5. My suggestion for the documents will have to be implemented. Some services could list as many as 10 documents, if they were in paragraph form they would take up less than 3 lines. Move "Civil" and "Fax" next to header "Documents". Then list documents with semi colon between each one. Example "Correspondence to Deponent; Notice of Deposition; Deposition Subpoena and Attachents3.
